Why does ChatGPT forget decisions the company already made?

ChatGPT does not forget decisions because it has a bad memory. It forgets because the decision was never stored anywhere the model reasons from. A decision discussed in a meeting, approved in a Slack thread, or corrected once in a chat does not automatically become part of how the workspace operates going forward.
Unless a correction is captured and fed back into the system, the next session starts from zero again.
